Willow's Heart is a quest item in Pathologic. It may be obtained in the Haruspex Route on Day 5 for the main quest "The Quivering Heart".

While searching for an infected heart to give to the Bachelor, the Haruspex may find Willow Mellow in Andrey Stamatin's Pub. After speaking with Willow, the Haruspex receives a letter from Anna Angel, asking him to kill Willow in exchange for a large sum of money.

Should the Haruspex choose to fulfill Anna's request, he can find Willow standing in the Blind Backyard behind Andrey Stamatin's Pub and kill her, after which he can obtain Willow's Heart from her corpse. The Haruspex can receive a reward from Anna Angel for killing Willow even without taking her heart. If he gives Willow's Heart to the Bachelor to inspect, the Bachelor will not provide a reward for it.
