Butcher's Heart is a quest item found in Pathologic that may be obtained during the Haruspex Route. It may be obtained on Day 5 by the Haruspex for the side quest "The Quivering Heart".
While searching for an infected heart to give to the Bachelor, the Haruspex may find a Worm herb gathering hut in the Steppe where an Herb Bride and several Butchers are also standing nearby. The Butchers will demand the Haruspex to kill the Herb Bride. The Haruspex can refuse to comply and choose instead to kill the Butchers. He can obtain up to three Butcher's Hearts from their corpses. When the heart is given to the Bachelor, he will provide the Haruspex with a large monetary reward and 2 bottles of the Black Vaccine.
